As the episode begins, both Croft brothers remain comatose and on life support. Watson holds the lone remaining dose of the life-saving treatment he developed. With no clear direction, he makes a coin toss. He injects the vector into one twin’s brain. Slowly, the chosen patient awakens.
“Where’s Stephens? Is he OK?” asks the newly conscious twin—Adams (Peter Mark Kendall). His question confirms who the coin chose. Though Adams is awake, Stephens remains in a coma. Time is running out for him, and even if he survives, the growing lesions on his brain threaten his future.
THE RETURN OF SHINWELL

Dr. Watson calls Shinwell (Ritchie Coster) and urges him to return, despite any guilt he may carry. Stephens needs him. Shinwell shows up at the hospital, visibly unwell, but determined. He proposes a bold move: leverage Moriarty’s pharmaceutical agent, played by Kacey Rohl, by offering her daughter treatment at the clinic.
The agent’s daughter, Ashleigh, suffers from a brain tumor. Shinwell negotiates with the woman, asking her to retrieve a sample of the antidote that Professor James Moriarty (Randall Park) used to induce Stephens’ coma. Though terrified, she agrees.
Ashleigh is just 9 years old. A chemo-based treatment offers hope with a 90% survival rate, but her current weight is too low to tolerate the regimen. Watson notes that they must find a way to help her gain at least 15 pounds. Dr. Laila Bynum, Ashleigh’s oncologist (played by Tika Sumpter), arrives, skeptical of Watson’s unconventional methods and clinic. She accuses him of giving false hope.
MORIARTY REACHES OUT TO INGRID
While Dr. Ingrid Derian (Eve Harlow) is attending her sister Gigi’s (Kiera Allen) physical therapy, Moriarty unexpectedly approaches her. He gives her a card, suggesting that they are no longer simply adversaries—they’re collaborators now. The implications are chilling.
Back at the hospital, Dr. Sasha Lubbock (Inga Schlingmann) visits Stephens’ bedside. She confesses her feelings with a kiss on his forehead. As if reacting to her touch, Stephens suddenly begins to seize. At the same time, Ashleigh experiences a seizure as well.
SEIZURES LEAD TO CLUES
Ashleigh’s seizure brings new understanding. Doctors determine that something other than her tumor is causing her weight loss. Sasha pressures her to talk. Ashleigh reveals that Moriarty won’t kill indiscriminately; he only targets “the right people.”
This information leads Watson and Shinwell closer to locating Moriarty’s secret lab. But when Watson examines Shinwell, he finds severe rashes on his back. Shinwell refuses treatment until Stephens is safe.
INGRID REVEALS EVERYTHING
Ingrid contacts Moriarty, informing him that the clinic suspects the location of his lab. She tells him to clear out vital equipment. Moriarty speaks of power, legacy, and domination, welcoming Ingrid to a select circle.
Meanwhile, Watson discovers that Ashleigh is allergic to ingredients in her nutritional shakes. A simpler, older formula might help her gain the necessary weight. The agent, whose name is Hannah (possibly Anna), prepares to enter Moriarty’s lab to steal the antidote. Before she can act, Moriarty’s henchmen arrive, tipped off by Ingrid.
Hannah narrowly escapes after a warning call from Watson. Relieved, she learns that Ashleigh will qualify for surgery once she gains weight. Watson dismisses her from the mission.
INGRID COMES CLEAN
Shinwell is confused by the sudden change. That’s when Ingrid confesses everything: Moriarty approached her; she tampered with the vector; and she murdered her father after he severely harmed her sister. Tears stream down her face as she admits that Stephens’s near-death condition is her fault.
Watson reveals that he already knew. Ingrid had confessed the previous night. Together, they devised a trap for Moriarty.
THE PLAN TO POISON MORIARTY
Ingrid explains that Moriarty, during their meeting, left behind a trace of his skin. Watson used it to concoct a custom toxin designed to affect only someone with Moriarty’s DNA. Ingrid went to the meeting place early and coated all surfaces with the substance.
Now Moriarty is infected.
“He knows I’m the only person who can save him,” Watson tells the team.
MORIARTY’S FINAL PLAY
Moriarty arrives, clearly suffering. He confronts Watson, disoriented and unable to see. Watson shows no sympathy. With Moriarty carrying the stolen vector, Watson and his team begin simultaneous procedures on both Moriarty and Stephens.
Stephens wakes up, clear-headed. His brother Adams is by his side, apologizing for dating Lauren. Stephens forgives him, saying he understands and wishes them happiness. Sasha visits next, reminding him that she kissed him while he was unconscious—then kissing him again.
Ingrid stops by, grateful but aware. Stephens thanks her for saving his life, even though she caused his illness. “I can’t work with you. You have to go,” he says. Heartbroken, Ingrid leaves.
THE END OF MORIARTY
With his vision restored, Moriarty finds himself face-to-face with Watson one final time. He questions why Watson saved him only to let him die later. Watson clarifies: the “cure” wasn’t truly a cure. It was part of the plan.
Moriarty realizes too late that Watson poisoned him under the guise of treatment. He accuses Watson of breaking the Hippocratic Oath.
“You are the harm, James Moriarty,” Watson replies. “I treated you, but I’m glad you won’t be walking out of here.”
Moriarty begins to seize. He gasps for air and struggles against the inevitable. “So you’re going to die with your secrets, eh?” Watson asks.
“What else do I have left?” Moriarty responds before he dies.
WATSON STARTS DATING DR. LAILA
Two weeks later, Watson enjoys a quiet evening with Laila. She’s cooking dinner at his apartment—their third date. Watson carefully folds Moriarty’s polo shirt, the one emblazoned with “Always & Everywhere,” and places it in a box marked “HOLMES.” He slides the box under his bed, closing the door on a chapter filled with danger, betrayal, and moral ambiguity.
The season ends not with fanfare, but with reflection. Justice, loss, and choices haunt every frame. Watson lives on—scarred but steadfast.
