Veronic assigns key cards to everyone and makes them work there. Walsh is now in charge of the ED, and Casey and Lauren must adjust. Max is impotent, and they’ve all signed a non-compete agreement that says they can’t practise medicine for three years or work at New Amsterdam. Max is adamant about not telling Helen the truth. Wlder and Floyd are caring for a pregnant woman. She’s having twins, but both of them have tumours, so the goal is to pull the twins out, remove the tumours, and reintroduce them. Helen enters and revives a child who has coded, and one of her colleagues tells her that she has simply made it more difficult for the family to say goodbye. Max ends up treating all of the hospital’s janitors. They remove the tumours from the infants, but the placenta shrinks, limiting them to only one baby, which they must choose. Bloom’s acceptance of him as her employer throws Walsh for a loop. The UK government steps in and announces that the child Helen will be taken off life support. The youngster Iggy was chatting to in the game says he’s the only person he trusts and that he wants to see him. Iggy recognises he’s deceiving him and that it’ll be a problem.

To save them both, Wilder advises making one of the babies an ectopic pregnancy. Helen informs Max that the young patient will be transferred to New Amsterdam. Max discovers that the entire medical staff is receiving substandard care and is suffering from health problems as a result of being overworked. The surgery is completed by Wilder and Floyd. The cops arrive as the youngster is being taken away. DHS immediately put a stop to everything and threatened to arrest Helen and her parents. Iggy introduces himself as a therapist to the youngster. The child vanishes. However, his next patient is the child with whom he was conversing. They come in seeking assistance. Everyone who assisted Max is denouncing her, and no one is cleaning up the OR or preparing anything. The board’s attention is drawn to the protesting critical workers, prompting Veronica to depart permanently. Everyone rejoices, and Karen wonders if Max will return.
